Care Navigator - Emergency Department
Bowling Green, OH
Wood County Hospital is currently accepting applications for a Care Navigator in the Emergency Department. This is a full time (36 hours per week) afternoons (10:45a - 11:15p) position. The Care Navigator provides facilitation of patient throughput and placement, either internally or externally.
Collaborates regularly with patient care provider and others to assist with compliance with regulatory, accrediting, and professional organization standards and requirements to improve the quality of patient care delivery.
Assists with conducting, analyzing, and acting upon information obtained during clinical interactions.
Assists with coordination of patient transfer needs.
Participates in patient care delivery either directly and indirectly.
Provides assistance for clinical decisions when Department Director and/or Unit Supervisor are not available.
Assists with medication reconciliation and home medication education.
Graduate of an accredited school of nursing.
Graduate of an accredited BSN program.
Current active RN license in Ohio.
Current certification (or obtained within 4 years in the position) from either the Commission for Case Management as a certified case manager (CCM) and/or from the National Association forHealthcare Quality (NAHQ) as a certified professional in healthcare quality (CPHQ).
Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.
Current American Heart Association (AHA) Advanced Cardiac Life Support (ACLS) certification required for Emergency Department required.
Current American Heart Association (AHA) Pediatric Advanced Life Support (PALS) required.
